Simple Proxy Suggestions
- Mark the box next to Brian Erhmantraut under step 1.
- Mark nothing else on page 1.
- Flip over the paper and sign and date
- Mail to PO Box 24.
If you need a new proxy form, you can click on the purple button. This will provide a PDF of a blank proxy form. You will need one form for each parcel that you have a EWUA membership for. You will need to find your account number. It is on your water bill or on your EYE ON WATER account.
You might need a blank form because you didn’t get one, you lost the one you received, you want to submit a new form to replace the one you sent in already. If you make this a replacement, make note on it that this replaces your prior submission.
The form is set to print on legal size paper. You can tell it to shrink to fit to 8.5×11. It is still readable. The form you received in the mail is double-sided. If your printer can print both sides, great. If not, the Inspector can still follow your instructions, though it would be wise to staple the two pages together.
